I don't think so. There was one in Larry Bate's list of items for sale a
while back with catalogue no. EGOD 21, but that fact that it's extremely
rare leads me to think that it was never released commercially, maybe just a
few pressed up as a test but not in commercial quantities.

Colesy wouldn't have one as he won't sully himself with anything he didn't
design the sleeve for ;>)
