It was certainly played pre-Extremities release, but since it surfaced at the same gigs as Age of Greed, Beautiful Dead, Intravenous, etc, I classed it with them. I'd guess that at the date of the gig, they were expecting it to go on the album.

Anyone *know* why it didn't?  I'd say it's as good or better than some of the tracks that did make it...
