My loudest gig was NEUROSIS in Hamburg, early nineties.
So loud I spent most of the concert crouched in a foetal (sp?) position rocking back and forth. Left when it was enough and I couldn't take any more. Good stuff.
Second loudest: Swans, also in Hamburg. Long ago. Wonderful...
Third loudest: Ministry, again in Hamburg, must have been the Psalm 69 tour.

The Academy in 91 was bloody loud . . Swervedriver supported didn't they?
I also remember Geordie being VERY loud with a wahwah pedal at Watford (94?)
Loudest gigs I have seen:
Damage Manual - Cambridge Junction (so loud they were almost hypnotic)
Motorhead at the old Marquee (Wardour Street) . . deaf for 3 days after
